چکیده

Direct hemoperfusion therapy with polymyxin B immobilized fiber cartridge (PMX-DHP) is an established strategy in the treatment of septic shock in Japan and parts of Western Europe. PMX-DHP is currently the subject of a pivotal North American randomized controlled trial (EUPHRATES) in patients with septic shock and confirmed endotoxemia, as measured by the endotoxin activity assay. The major mechanism of action of this therapy is the removal of circulating endotoxin. High affinity binding of circulating endotoxin by the PMX-DHP column may decrease circulating endotoxin levels by up to 90% after two standard treatments. Basic research has shown reductions in circulating cytokine levels and in renal tubular apoptosis. Clinical research has shown that PMX-DHP therapy results in hemodynamic improvements, improvements in oxygenation, renal function, and reductions in mortality. Further research is needed to further define additional patient populations with endotoxemia that may benefit from PMX-DHP therapy as well as to further elucidate dosing, timing, and additional information on mechanisms of action. This review will present the mechanistic rationale for this targeted strategy of endotoxin removal using PMX-DHP in endotoxemic septic patients, highlighting both the specific effects of the therapy and the evidence accumulated so far of clinical improvement following this therapy in terms of recovery of organ function.

The effect of endotoxin adsorber hemoperfusion on microcirculation in patients with severe sepsis and septic shock

Introduction Microcirculatory dysfunction may result in multiple organ dysfunction during severe sepsis and septic shock. (1) A meta-analysis of randomized trials showed that polymyxin B hemoperfusion (PMX-HP), plasma exchange, and hemofiltration were associated with lower mortality. (2) Mesenteric microcirculation was better maintained with polymyxin B hemoperfusion in one rat sepsis study. (3...

Effects of Polymyxin B Hemoperfusion on Mortality in Patients With Severe Sepsis and Septic Shock: A Systemic Review, Meta-Analysis Update, and Disease Severity Subgroup Meta-Analysis

OBJECTIVE Several studies have reported a survival benefit for polymyxin B hemoperfusion treatment in patients with severe sepsis and septic shock. However, recently, a propensity-matched analysis and a randomized controlled trial reported no survival benefit for polymyxin B hemoperfusion treatment. We performed an up-to-date meta-analysis to determine the effect of polymyxin B hemoperfusion tr...

Polymyxin B-immobilized fiber column hemoperfusion therapy for septic shock.

Endotoxin, an outer membrane component of gram-negative bacteria, plays an important role in the pathogenesis of septic shock. Endotoxin adsorption therapy by polymyxin B-immobilized fiber column hemoperfusion (PMX) has been used for the treatment of septic shock patients in Japan since 1994. The covalent binding of polymyxin B onto the surface of the polystyrene-based carrier fiber in PMX inac...

The effect of endotoxin adsorber haemoperfusion on microcirculation in septic pigs

Introduction Microcirculatory dysfunction plays an important role in sepsis-related multiple organ dysfunction.(1) Several studies has shown polymyxin B hemoperfusion has favorable effects on mean arterial pressure, vasopressor use, and mortality.(2) One rat sepsis study had found that microcirculation was better maintained in the polymyxin B hemoperfusion group.(3) However, the effects of poly...

Polymyxin B Hemoperfusion for Septic Shock -- Ready for Primetime? Early Use of Polymyxin B Hemoperfusion in Abdominal Septic Shock: The EUPHAS Randomized Controlled Trial

Background: Treatments for severe sepsis are limited, and its morbidity and mortality, especially from Gramnegative organisms, continues to be significant. Gram-negative organisms release endotoxin, which initiates inflammatory and coagulation cascades, resulting in end-organ dysfunction in patients. Polymyxin B hemoperfusion filters bind endotoxin. Objective: To assess the efficacy of polymyxi...
